Life! Program
Life! is a free healthy lifestyle program designed to help you:
Improve your eating habits
Increase you physical activity
Manage stress more effectively
The program is funded by the Victorian Government and managed by Diabetes Victoria.
Delivered by expert health professionals, Life! is offered as a group course, available face-to-face or online. The program supports eligible Victorians to reduce their risk of developing type 2 diabetes, heart disease, and stroke.
Note: The Life! program is not available for people who are already living with diabetes.
Eligibility
You may be eligible for the program if you meet one of the following criteria:

Australian Diabetes Risk Score (AUSDRISK) ≥ 12, and
BMI ≥ 25 (≥ 23 for Aboriginal and Torres Strait Islander peoples and people born in Asia)

Cardiovascular Risk Score ≥ 10 (can be calculated by your GP)

Cardiovascular disease
Gestational diabetes
Moderate or severe kidney disease
Prediabetes
Fasting blood glucose: 5.5–6.9 mmol/L
2-hour oral glucose tolerance test (OGTT): 7.8–11.0 mmol/L
HbA1c: 6.0–6.4% (42–46 mmol/mol)
Familial hypercholesterolemia
Serum cholesterol > 7.5 mmol/L
Systolic blood pressure ≥ 180 mmHg or diastolic ≥ 110 mmHg
